Approach to care
The Nourish program typically begins with weekly 55-minute sessions, over time, session frequency can be reduced depending on progress. In follow-up sessions, we will work together to implement the care plan we discussed. I will keep providing recommendations and resources and we will monitor progress toward your goals and set new goals. The total length of the program varies depending on the progress we make, but we typically see the best results after at least 12 sessions. Throughout the program, you will have access to the Nourish app on your phone, which will help us work together to track and hit your health goals. Does that sound good to you? Alright, let's do it!
Your first session with Jane
My specialty lies in delivering a patient-centered care experience. In our first session, I will perform a Comprehensive Nutrition Assessment. I will ask numerous questions to fully understand your lifestyle habits, medical history, and nutritional needs. This will help us develop a personalized plan by the end of the session to achieve your health goals. Additionally, we will set both short-term and long-term goals based on this assessment. Does that sound good to you?
